The agency shall maintain a record on the execution or attempted service of legal process documents that includes:

1. a.date and time service was executed/attempted;
2. b.name of officer(s) executing/attempting service;
3. c.name of person on whom legal process was served/executed;
4. d.method of service/reason for non-service; and
5. e.address of service/attempt.
