Standard statement

The agency shall have a written directive requiring that information regarding each item of criminal and civil process be recorded, to include the following elements:

Each bullet must stand alone when establishing compliance, with proofs identified for every lettered requirement.

1. a.date and time received;
2. b.nature of document;
3. c.source of document
4. d.name of defendant;
5. e.officer assigned for service;
6. f.date assigned for service
7. g.court docket number; and
8. h.date process expires.
